Concern for Independent Living provides contracted case management services for Long Island veterans with HUD-VASH Vouchers. This program utilizes a Housing First approach to ensure rapid rehousing and stabilization within the community. Concern Case Managers provide services be to assist in the expedited placement of HUD-VASH veterans and their families into permanent, supportive housing and to assist them in maintaining their housing and gaining as much independence as possible. Services are coordinated with other medical, psychiatric, educational, and vocational providers.
